DAVIS MIDDLE SCHOOL PTO
Thank you for your interest in joining the Alfonza W. Davis Middle School PTO. Please fill out the form below and return it to the office. Dues are $10.00 per person. This will allow you to put up to 3 e-mails per membership to receive PTO info. Please make checks payable to Alfonza W. Davis Middle School PTO. Membership fees and donations are tax deductible due to our 501(c)3 status.

In order to maintain and ensure the best possible academic environment at Davis, we rely on parents to take an active part in the activities and events that support our staff, students, and families. Some of our planned events are below.
I would be willing to serve on a planning committee and/or help volunteer at some of the following events:
Glow Run _______ Staff vs Student Basketball Game ______ Bake Sale ___________ 8th grade graduation __________
Hy-Vee Receipts/Box tops _________
Staff Meals for Conferences/Teacher Appreciation ________

I am happy to donate my time by volunteering to staff the bookstore (sign up sheets available at school registration night in August)
My time is limited, but I am willing to make a $40 donation (includes membership) to the PTO (check enclosed)

**New Volunteer Incentives: Help with 5 or more events you will receive at $12 bookstore voucher, 10 or more events = $30 bookstore voucher or attend 5 or more PTO meetings = $12 bookstore voucher. "Punch cards" to keep track of volunteer times will be available at school registration nights in August.
**All donations to the Davis PTO are tax deductible due to our 501(c)3 status

The bookstore will be open on the following dates:
August 29th 7-8pm PTO meeting
August 30th Open House 6-8:30pm
September 2nd First Friday 11am-1pm
October 13th Fall Play 6-8pm
October 20th Conferences 12-7pm
December 2nd First Friday 11am-1pm
(perfect time to purchase a holiday present)
